Participation waiver & medical release

I am the parent or legal guardian of the player(s) named below. I understand that basketball training involves physical activity and the risk of injury. I voluntarily allow my player to participate and release the organization, its coaches and partners from liability for injuries arising from ordinary participation, except where caused by gross negligence.

I confirm my player is medically able to participate. In an emergency, I authorize coaches to seek medical treatment for my player if I cannot be reached.

I understand sessions are non-refundable inside the cancellation window shown at booking and that credits are issued for timely cancellations.
